Losing your basic health-care plan with ONVZ
If you receive a pension or benefits from the Netherlands and you move to an EU/EEA or treaty country or Switzerland, you will not be able to keep your Dutch basic health-care plan. This also applies to your co-insured family members.
As soon as you know when you will be moving, you need to tell us. We will use the details you provide to determine the end date of your basic health-care plan. Please send us the following:
- proof of deregistration with your local council (or proof of registration in your new country of residence)
- your address in your new country of residence
Taking out a new health-care plan
You can take out a (new) health-care plan in the country you are moving to. If you are moving to an EU/EEA country, a treaty country or Switzerland and you do not yet have an S1 form (formerly E121), please request one from the CAK.
Use the S1 form to register with a health insurer in your new country of residence. By using this form, the following will be taken care of:
- you will not have to pay statutory health insurance premiums in your new country of residence
- you will be entitled to medical care pursuant to the legal regulations and rates of that country
- the CAK will collect your contribution for your new health-care plan
- you will receive a ‘Medical care entitlement certificate’ in the Netherlands from Zilveren Kruis. This certificate gives you the right to medical care during a temporary stay in the Netherlands
