Still working in the Netherlands

If you are moving abroad, this may affect your basic health-care plan. You should therefore check the consequences for you before moving.

  • If you continue to work in the Netherlands, you must have a basic health-care plan in the Netherlands

What is an S1 form?

The S1 form (formerly E106) is proof that you are entitled to medical care pursuant to the legislation and rates in your country of residence and that you pay your premium in the Netherlands.

If you live in an EU/EEA or treaty country, your Dutch basic health-care plan may not cover all health care in your country of residence, or may not fully cover it. One example is if health care in your country of residence is more expensive than in the Netherlands. With an E106/S1 form:

  • you can register with a health insurer in your country of residence
  • you can prove that you are insured with ONVZ and pay your premium to us
  • ONVZ will reimburse medical costs in your country of residence based on the rules of that country’s health-care system. Usually you won’t have to pay any (additional) premium for a health-care plan in your country of residence

If you need an S1 form, you can request one from the Service Centre.

Your family members

If your family members who are moving with you do not work in the Netherlands:

  • they will lose their Dutch basic health-care plan

In most cases, you can use the S1 form to register your family members with the health insurer in your new country of residence. This health insurer will determine who is co-insured and will notify the CAK of this. Usually you and your co-insured family members will then receive a health-care card from that health insurer or proof of registration. This will give you the same health care as residents of your country of residence who are insured there.

Registration of your family members will not be final until your health insurer notifies the CAK of this. From then on, your family members will be registered with the CAK as being co-insured with you. You will also receive a letter from the CAK about this.
