Are you coming to live or work in the Netherlands? And do you want to take out health-care insurance with ONVZ? If so, in our capacity as administrators of the Dutch Health Insurance Act, we are required to ask you for an employer’s or accountant’s statement. This is to ascertain whether you are covered by the Dutch health-care system and if so, from which date. 

The Health Insurance Act determines whether you are entitled to Dutch basic health-care insurance. It depends on the country in which you work or pay income tax. The date on which you started working or paying income tax must also be clear. Your employer or accountant can give us this information by supplying an employer’s or accountant’s statement. We use this statement to decide whether you must take out Dutch basic health-care insurance, and if so, from which date.
